How to Register for the Patient Portal

1. After your hospital or clinic visit (Rock Port, Tarkio or Fairfax), you will receive an email invitation to create your patient portal account. To start the registration process, click on the link provided in the email.
2. Enter your first name, last name and date of birth in the fields provided. Your profile number and email address should automatically populate. Next, create a unique username and password. When you are finished, check the box verifying you have entered correct information and click Register.
3. Select three security questions and provide your answers. These will be used to verify your identity if you lose or forget your username and/or password.
4. Select an avatar for your account.
5. Click on Medical Record to see your patient care summary.
6. Select the patient you would like to view. (If you are a Patient Portal Authorized Representative for more than one patient, you will see all of them listed here.
7. Select the account you want to view. All accounts will be listed by visit number, admin date and discharge date.
8. Select the patient care summary you would like to view. The summary will include information including, but not limited to, laboratory test results, medications, allergies, immunizations and medical conditions.
9. To view another account, select Clinical Information.

Health Records on iPhone

We are excited to announce that Community Hospital-Fairfax supports health records in the Health app, which is available to patients with an iPhone running iOS 11.3 or later.
• The new health records feature in the Health app makes it easy to access and visualize key parts of your health records, including allergies, conditions, immunizations, labs, medications, procedures, and vitals.
• Setup is simple. Getting your health records from Community Hospital-Fairfax into your iPhone is as easy as going to the Health Records section of the Health app, searching for our institution, and then logging in with the user name and password that you normally use to access your account at our institution. If you don’t already have a user name and password, contact CH-F Medical Records at (660) 686-2345.
• After you log in once, your health records will start to appear in the Health app, and you will be notified when new records are ready to view.
• Your health record data is kept secure, because it is encrypted and protected with the passcode on your iPhone.

What is the Patient Portal?

The Patient Portal is an online health management tool that includes a view of clinical data. This access provides you with information, including but not limited to, the following:

• Laboratory Test Results

 Who is eligible for the Patient Portal?

All patients 18 years of age and older or otherwise legally eligible receiving the following services:

• Inpatient

• Inpatient Surgical

• Obstetrical

• Skilled (Swingbed)

• Observation

• Ambulatory Surgery

• Select Clinic Visits

How do I set up an account?

The email address you shared with us during the registration process will receive an auto-generated email from our electronic health record. Once you have completed the initial setup of your account, you will only need your unique username and password to sign into the Patient Portal in the future.

How do I access the Patient Portal once I have completed the account setup process?

For future visits to the Patient Portal, visit the login page at: https://www.thrivepatientportal.com/

Remember, use this link AFTER you have received a portal invite and completed the sign-up process.

Will I receive emails after every admission to the hospital or clinic visit?

No. After each visit, a new summary of care document will automatically post to your Patient Portal account. You may access the document any time after you are discharged. Once the initial invitation email has been sent and your account has been created, you will not receive emails after each visit.

Who else can see my personal health information or access my Patient Portal?

You, anyone you provide your User ID and Password to, and anyone you designate as your Patient Portal Authorized Representative will have access to your Clinical Information. Your Patient Portal Authorized Representative will only have access to your Patient Portal. They do not have access to medical records and cannot make medical decisions on your behalf. 

If you think someone has gained inappropriate access to your account, you should immediately change your password on the portal site.

How do I change my password?

After logging into your account, select settings. Then, select security in the upper left corner. Enter your old password and create a new password. Select “Update Password” when you are finished.
