New Electronic Health Record Coming This Fall at Atchison County Family Medicine Clinics

Dear Clinic Patient,

We are very excited to announce that we are implementing a new electronic medical record at Rock Port Family Medicine Center, Tarkio Family Practice and Fairfax Family Medicine Clinic. This is the first exciting step to a system wide electronic medical record which we believe will bring additional continuity, efficiency and safety to your care.

To make our electronic medical record as effective for our patients and caregivers as possible, we are carefully building an individualized health profile for each of our clinic patients. When complete, our providers will have more information than ever before at their fingertips as they make decisions about your care, information will flow seamlessly between the clinics and hospital and, in the future, a patient portal will become available to you!

We need your help as we build your individualized health profile. Linked below and in the side bar is a Patient Intake Form. We would appreciate it if you, or the person in your life assisting you with healthcare decisions, would complete and return this form to the hospital or your local clinic as soon as possible. This will help us build your profile and save you time at your next primary care visit.

If you would rather share this information via phone, simply call (660) 686-2211 ext. 310. You may also stop by the hospital for assistance between the hours of 6 a.m. and 4 p.m. Monday through Friday.
